Who can participate?
- Participants, Families, and Caregivers

- Family and caregivers include an adult child, spouse, partner, sibling, neighbor, or other unpaid support adult designated by the older adult.
- Patients that are receiving rehabilitation services from either a skilled nursing facility or home health agency.
What can participants expect?
- Expectations

- Participants will engage in a single phone interview lasting 30-45 minutes.
- In appreciation of your time, you will be offered a $50 e-gift card.
- Participation is confidential and completely voluntary. You can withdraw at any time.
