Implementation of Centers for Medicare & Medicaid Services post-acute care payment policy changes

• Patient Driven Payment Model (PDPM) in October 2019
• Patient Driven Groupings Model (PDGM) in January 2020

Emergence and evolution of the COVID-19 pandemic

• Exploring staff and leadership experiences of working in home health and skilled nursing
• Examining the experiences of patients, families and caregivers navigating the healthcare environment
• Understanding the reality of care delivery in a post-COVID-19 emergence world

This project wants to learn from skilled nursing facility and home health agency leaders, staff, patients, families, and caregivers.

Study Objective

To capture the voices of skilled nursing facility and home health agency leaders, providers, patients, families, and caregivers.
